  Nursing Home Transition and Diversion  
 

CIDNY is the Regional Resource Development Center for the Nursing Home Transition and Diversion Waiver.  This is a program that allows people on Medicaid who are living in nursing homes to return to the community with a wide range of services and also helps people at risk of nursing home placement to remain in their own homes.  There are 17 services available under this Waiver including a rental subsidy.For further information:
